Our commitment

Proximatrix LLC takes the confidentiality of your health information seriously. Where the practice operates as a covered entity under the Health Insurance Portability and Accountability Act (HIPAA), it is required to maintain the privacy of protected health information (PHI), provide this notice, and follow the terms of the notice currently in effect.

This document is a template. Whether and how HIPAA applies depends on the practice's specific circumstances, including electronic transactions, business associate agreements, and administrative and technical safeguards actually in place. No claim of compliance is made here.

Uses and disclosures without your written authorization

• Treatment — coordinating your care, including with other providers you authorize.
• Payment — billing and collecting fees for services provided.
• Health care operations — quality review, training, and administration.
• Required by law — including mandated reporting of suspected abuse or neglect of a child or vulnerable adult, court orders, and public health requirements.
• Serious threat to health or safety — disclosure necessary to prevent or lessen an imminent, serious threat to you or others.

Uses that require your written authorization

• Most uses and disclosures of psychotherapy notes.
• Use or disclosure for marketing purposes.
• Any sale of protected health information.
• Other uses not described in this notice.

You may revoke an authorization in writing at any time, except where we have already acted in reliance on it.

Your rights

• Inspect and obtain a copy of your record, subject to limited exceptions.
• Request an amendment to information you believe is incorrect or incomplete.
• Request an accounting of certain disclosures.
• Request confidential communications by alternative means or at an alternative location.
• Request restrictions on certain uses and disclosures.
• Receive notification of a breach of unsecured protected health information.
• Obtain a paper copy of this notice on request.

Complaints

If you believe your privacy rights have been violated, you may complain to the practice using the contact details below, or to the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.

Changes to this notice

We may change the terms of this notice and make the new notice effective for all information we maintain. The current notice will be posted on this page.
