Not an emergency service

This website and its forms are not monitored continuously and must not be used for emergencies. In an emergency call 911, or call or text 988.

Appointment requests

Submitting a request does not create a confirmed appointment. Appointments are confirmed only when the practice contacts you. We may decline or reschedule requests, including where a service is not clinically appropriate or where a client is not located in a state where our clinicians are licensed.

Fees and payment

Fees are as published on the Fees & Payment page and are subject to change with notice. Payment is due at the time of service unless agreed otherwise. Late cancellation and no-show charges apply as described in the Cancellation & No-Show Policy.

Your responsibilities

Provide accurate contact information.
Do not submit confidential clinical information through unsecured web forms.
Do not use the site unlawfully or attempt to disrupt its operation.
