Telehealth Consent
Please review this information before your first online session. You will be asked to sign a telehealth consent as part of intake.

What telehealth means here
Telehealth is the delivery of counseling services using live, two-way video and audio instead of an in-person visit. Sessions are held on a platform selected for security and confidentiality.
Potential benefits
- Access to counseling without travel or time away from home or work.
- Greater scheduling flexibility, including in poor weather.
- Continuity of care when attending in person is not practical.
Potential risks and limitations
- Technology can fail; a session may be interrupted or need to be rescheduled.
- Video may make it harder to observe non-verbal cues.
- Despite security measures, no electronic transmission can be guaranteed fully private.
- Telehealth is not appropriate for acute crises or situations needing in-person assessment.
Your responsibilities
- Be physically located in New Hampshire for each session unless otherwise agreed in writing.
- Use a private space where you will not be overheard or interrupted.
- Provide your current physical address and an emergency contact at the start of each session.
- Do not record sessions; your counselor will not record them either without written consent.
- Avoid attending sessions while driving or operating machinery.
If the connection drops
If a session is interrupted, your counselor will attempt to reconnect. If that is not possible, they will contact you by telephone at the number on file to continue or reschedule.
Emergencies during telehealth
Because your counselor may be physically distant, emergency response is limited. If you are in danger, call 911 or call or text 988. Your counselor may contact emergency services or your emergency contact if they believe you are at serious risk.
Alternatives and withdrawal of consent
In-person sessions and referral to other providers are available alternatives. You may withdraw consent for telehealth at any time without affecting your right to future care.
Fees
Telehealth sessions are charged at the same rate as comparable in-person sessions, and the same cancellation terms apply.
